Email us at h2ofit@rogers.com, indicating whether you wish to sign up for one class per week ($29/month), or unlimited classes per week ($59/month).

Print and complete the following (Note: you must save the forms to your computer first if you wish to use the fillable form fields.)

 

Scan and email us the completed forms and cheque marked "void", or
